Delivery Room CPAP or DR CPAP is the use of Continuous Positive Airway Pressure therapy immediately after birth in the delivery suite or labour ward. This positive pressure helps with alveolar recruitment and forming the functional residual capacity, supporting a baby who must transition from fluid filled lungs in the womb to air filled lungs after delivery.
The NeoFlow® Delivery Room CPAP circuit and accessories can seamlessly integrate with any standard resuscitator to provide CPAP in the Delivery Suite, requiring no specialised CPAP machines. NeoFlow® supports Family Centred/Integrated Care, with a longer circuit length option available, baby can receive cuddles whilst receiving the required respiratory support.
Humidified delivery Room CPAP to heat and add water vapour to the gas flow from the resuscitator using a specialised heated circuit is also an option, for most labour wards and delivery suites a heater, humidifier can be added to the side of the radiant warmer or mounted by the resuscitator on a receiving trolley.
Early CPAP can reduce the need for a baby to be intubated. A Cochrane Review comparing the use of nasal CPAP, to mechanical ventilation shows that there is a reduction in the need for intubation and surfactant, lowering the risk of BPD as CPAP minimises the risk of lung injury by avoiding barotrauma and volutrauma that can be associated with traditional mechanical ventilation.
Once the NeoFlow® CPAP system is secured and baby is stable, they can be passed back to their parents for cuddles. The fact that PEEP is being delivered via a dedicated CPAP interface means a face mask does not have to be held in place to maintain a seal and deliver pressure. NeoFlow® DR CPAP circuits allow PEEP to be delivered even be provided while the baby is receiving delayed cord clamping.
Studies have shown that allowing the baby to aerate their lungs before the cord is clamped, which may require some PEEP support, allows for a more physiologically stable transition after birth.
